Support Patients in Maintaining Daily Function at Home

@Home Therapy is seeking a Certified Occupational Therapy Assistant (COTA) to provide in-home therapy services to adult patients across the community. In this role, you will assist in delivering treatment plans designed to improve independence in daily living activities and support functional recovery in real-world environments.
This is a field-based opportunity ideal for clinicians who prefer hands-on patient care in a flexible setting.

What You’ll Be Doing:

Implementation of Therapy Plans:

• Carry out treatment plans developed by the supervising Occupational Therapist.
• Support patients in practicing functional daily living skills. 

Hands-On Functional Support:

• Assist with self-care tasks such as dressing, grooming, and routine household activities. 
• Reinforce safe movement and activity techniques in the home setting. 

Patient Observation & Reporting:

• Monitor patient response to therapy and report progress to the supervising OT. 
• Communicate any functional changes or concerns. 

Caregiver Instruction Support:

• Reinforce caregiver education provided by the supervising clinician. 
• Help ensure consistency of therapy strategies between visits. 

Documentation;

• Complete visit notes using a digital EMR system. 
• Maintain accurate and timely records in compliance with regulations.

Qualifications:

• Active New York State COTA license.
• Experience with adult or geriatric populations preferred. 
• Ability to follow treatment plans accurately and independently. 
• Strong communication and interpersonal skills. 
• Comfort using electronic documentation systems.
